LCHPS Outreach

We are here for you.

Through understanding, lived experience and compassion, we can assist you in navigating various resources and helping you access the supports

you feel you need.


It's not about what you have done, what substances you use, or even how much you use...

We are a non-judgmental, safe place to talk.

Come to our office, meet our Outreach Worker at a local coffee shop, or arrange a visit in your home.


Wherever you are at, we can offer something to you.

How Our Outreach Model Works

The following process illustrates how outreach and our program works.

We operate from a Continuum of Care Model, meaning we'll meet you where you're at and continue to stand by you, advocate for you and support you in your journey.


We are here for you.

1. We Answer

Sometimes reaching out is the hardest step.

We get it.

So, you can call, text, send us an email or

drop by our office...

We're happy to communicate in whichever way works for you.

2. We Listen

Connect with Stefanie, our LCHPS Outreach Worker and have a chat about what you're looking for.

Wherever you're at in your life and substance use, we'll meet you there.

We offer a supportive, compassionate and non-judgemental ear.

3. We Provide

We'll provide you with resources and service referrals relevant to you.


We won't assume anything about your life or suggest certain services or resources over others.


We're just here to help you make the connections you want. 

We've done the research and have lots of details about service locations, hours, intake processes, parking, fees etc.

4. We Continue

Our Outreach approach is more hands on than just handing over a pamphlet and sending you on your way.


We see this as a journey, not a destination.

We'll continue to support you, and help you update and adjust your plan as things change.
